Skilled Nursing

Our skilled nurses provide comprehensive clinical services including wound care, infusion services, medication management, patient education to promote self-management, disease and pain management and cardiac and pulmonary care.

Physical Therapy

Our physical therapists help patients recover in the home from a variety of surgeries or illnesses. These services help patients become independent following an injury or illness. Our physical therapists specialize in total joint replacements, broken bones, stroke, back surgeries, Parkinson’s Disease, and many other musculoskeletal and neurologic disorders.

Occupational Therapy

Our occupational therapists assess and treat patients in the home in order to assist them in the development and execution of activities of daily living.  With a focus on adapting the environment to fit the patient, our occupational therapists excel at getting our patients to meet their goals within the home.

Speech Therapy

Our speech therapists assist patients in the home with a variety of speech and swallowing issues.  Suburban Home Health’s speech therapists have extensive knowledge and training to assist patients with cognitive issues. Speech therapists also educate caregivers on signals and warning signs of Alzheimer’s, and provide dementia training as well.

Medical Social Services

Our medical social workers connect patients with the appropriate community resources to assist with a variety of patient concerns. In collaboration with other clinicians, the medical social worker assesses the patient’s needs while acting as a patient advocate to suggest appropriate assistance programs and resources.

Home Health Aide

Our home health aides assist patients with personal care needs and activities of daily living in the home. Under the direction of a registered nurse, the home health aide supports patients in bathing, dressing and light meal preparation.


Effectiveness

Since your assigned home health care provider builds a relationship with you and works with all of your doctors as part of a collaborative team, you’ll see better results more quickly with home health care. Your provider will carefully monitor your condition and progress, making changes to your individualized care plan as needed to ensure your success. Patients who receive home health care dramatically lower their risk of re-admittance to the hospital or a rehabilitation facility.

Because it’s not around-the-clock care and because you are cared for in your own home, costs are significantly reduced compared to long-term rehabilitation center residence or hospitalization. At the same time, the care you receive from your highly skilled and trained home health care provider will be individual, focused, and carefully coordinated with other doctors to prevent duplication or redundancy.

Results

Home health care is available for homebound individuals recovering from illness or injury, or who need help getting started in self-management of ongoing medical conditions. The focus of home health care is on recovery, education, and healing, and so most patients remain in a home health care program for around 60 days

For over 30 years, our mission has been the same—to serve our community by providing exceptional medical care and positive outcomes to homebound patients. Our agency first formed as Hancock Regional Home Health, and gradually expanded to serve an ever-widening array of counties in central Indiana.

Currently affiliated with Hancock Health, Riverview Health, Witham Health Services, Margaret Mary Health, Major Health Partners and Hendricks Regional Health, Suburban Home Health works as the community care arm of those respected hospital systems, while also serving patients from other hospital systems and practices in the area.
